Health Navigation is an intervention model developed by Pacific Clinics in collaboration with University of Southern California. Health Navigation Skill Development Certification Training trains peer advocates, community health workers and medical caseworkers to assist individuals in navigating the healthcare system and building the skills and confidence to self-manage their health care. After completing a specialized 35-hour training each certified Health Navigator works with individuals to “bridge” the mental health, primary healthcare, and specialty healthcare systems.
The training program includes a Health Navigator Intervention Manual which describes in detail the skills needed to serve as a Health Navigator, including but not limited to: Screening, Engagement, Evaluation, Goal Development, Healthcare Navigation, Motivational interviewing, and Clinic Integration. There is also an option for add-on coaching sessions to reinforce the skills learned.
