Support Services offer various programs to address social determinants of health, including housing and employment coaching and placement, among other critically needed services.
Children and young adults ages 0 to 20 experiencing emotional and behavioral challenges deserve support. Whether a youth is at risk of psychiatric hospitalization, needs crisis support, or is at risk of dropping out of school, we are here to help.
The Success First/Early Wraparound program at Pacific Clinics provides resources for youth in crisis to help them manage life’s ups and downs and build resilience.
Through our program, we work with the youth and their family or caregiver to provide immediate support and create a long-term care plan that makes sense for them.
When youth enter our program, they can expect to be with us for four to six months before moving on to a longer-term solution. By building on their strengths, we aim to help them feel stable and confident.
Success First/Early Wraparound serves children and young adults eligible for Medi-Cal. The program assists youth facing emotional and behavioral difficulties, such as, or because of:
Many of the youth we serve deal with instability in their daily lives and don’t know who to turn to for mental health help. Our clinicians are on-call 24 hours daily to provide intervention.
Our crisis support program will help stabilize the youth and help manage complex emotional and behavioral challenges.
When a youth is discharged from a crisis center or hospital, they can find help at Pacific Clinics. Their parent or caregiver can bring them in, or a hospital or mental health facility can refer them to our program.
Once a youth is referred, we’ll assess them to determine their needs. In collaboration with families, we’ll develop a care plan that provides the best support and individualized care.
We’ll also invite the client’s input every step of the way. We find that care is not a one-way street, and by actively contributing to developing their care plan, clients will recover more quickly and confidently.

As a youth moves through our program, we’ll work with them to improve unhealthy behaviors. Family involvement and treatment that emphasizes their unique values and strengths can help them overcome these hurdles and get back to living a healthy and sustainable lifestyle.
After spending four to six months in our Success First/Early Wraparound Program, youth will feel more stable, and they can graduate to a lower level of care with a long-term provider, if support is still needed.
