Hope Program and Katie A Program
Support Services offer various programs to address social determinants of health, including housing and employment coaching and placement, among other critically needed services.
The Capital Region’s Transitional Housing Programs (THP) support young people transitioning out of foster care or juvenile probation by providing life skills coaching, essentials, and safe housing. Through one-on-one training and mentorship, THP helps clients work toward independence, fostering academic or employment success and lasting family connections.
The goal of each of the Capital Region’s Transitional Housing Programs (THP) is to provide support to those transitioning out of foster care or juvenile probation, and assist in their journey to independence by providing life skills coaching, basic necessities and more, all in a safe, housed environment.
Each client is served through one-on-one training in shared housing units leased by our organization while having the support of supervision, mentoring and a safety net while they work toward academic or employment goals.
Along with coaching life skills for successful living after emancipation, we assist clients in establishing and maintaining lifelong connections with family members or families of choice.
