Hope Program and Katie A Program
Support Services offer various programs to address social determinants of health, including housing and employment coaching and placement, among other critically needed services.
The Hope program and Katie A program provide individualized mental health support for children, teens, adults and families across the Santa Clara and Fresno counties areas to strengthen family connections, reunify families and help them overcome challenges.
Families across Santa Clara and Fresno counties have diverse needs and face distinct obstacles that sometimes disrupt the family environment. At Pacific Clinics, we provide integrative behavioral health care and social services to youth and families of diverse ethnic and socioeconomic backgrounds. We aim to help families stay connected, stabilize home placements and overcome the unique mental health or behavioral challenges that threaten their homes.
We work closely with the Department of Social Services and the Department of Behavioral Health to provide multi-faceted support, embrace a strengths-based model and bolster family connections.
Through the Hope program, our clinicians provide specialized mental health support to families with open Child Protective Services/Department of Social Services (DSS) cases. The goal of this program is to make recovery possible for youth in our welfare system and provides mental health support, skill building, behavioral coaching, and other support services to improve mental health outcomes within the family.
When teens and families enter the Hope program, we assign them a team that consists of clinicians, psychiatrists, nurse practitioners, and/or additional supporters. The team partners with the family to create an individualized treatment plan, which may include:
Many families receive specialized cross-system mental health support through our Katie A program. In conjunction with the Santa Clara County Department of Family and Children’s Services (DFCS), Katie A provides community-based services and supports for children and families.
We commonly work with youth experiencing mental health or behavioral challenges, along with their families, to stabilize the home environment and support family reunification. We also support teens experiencing challenges within their relative or foster care placements.
The Katie A program includes intensive mental health services and connects families with community resources. We facilitate Child and Family Team meetings to coordinate efforts between the family, the child’s school staff, the DFCS, Juvenile Probation Department (JPD) and other community partners.
