CAMPBELL, CA, May 21, 2020 – Uplift Family Services is pleased to announce the receipt of a two-year $4,000,000 grant from the Substance Abuse and Mental Health Services Administration (SAMHSA), a federal agency with a mission to reduce the impact of substance use and mental illness in the United States.
As part of the grant, Uplift Family Services, in partnership with School Health Clinics of Santa Clara County and Pacific Clinics, will form a consortium to become a Certified Community Behavioral Health Clinic (CCBHC). The CCBHC will serve 2,000 low-income individuals in Santa Clara County by offering access to and improving the quality of mental and substance use disorder treatment in the community by integrating physical health services.
The consortium is among 200 behavioral health organizations and health centers in the nation, and one of five in California, to become a CCBHC.

Together, these collaborating agencies will offer a wide array of services:
- Uplift Family Service will co-locate in School Health Clinics of Santa Clara County’s clinic to provide crisis mental health services; trauma-informed and evidence-based mental health and substance abuse treatment; psychiatric services, including consultation to primary care physicians; peer and family supports; systems navigation support; and vocational and educational counseling.
- School Health Clinics of Santa Clara County will provide outpatient primary care screening and monitoring of key health indicators; vaccinations where indicated; clinical monitoring for adverse effects of medications, including monitoring for metabolic syndrome; and case management. Services will also include provision of appropriate psychotropic medication inclusive of long-acting injectable anti-psychotic medication and FDA-approved medication treatments for substance use disorders including tobacco, alcohol, and opioid use; and screening for HIV and viral hepatitis (A, B, and C).
- Pacific Clinics will offer various remote services including telepsychiatry; consultation and training with prescribers and nurses on substance use and medication-assisted treatment; training certification for peer partners, which will educate individuals with lived experience to support clients; and technical assistance in intensive adult programming.
