Support Services offer various programs to address social determinants of health, including housing and employment coaching and placement, among other critically needed services.
The emotional, physical and intellectual environment that children experience before age five profoundly impacts all aspects of their development. Pacific Clinics’ First 5 program works with families to raise children who are healthy, emotionally adjusted and ready to learn in preparation for kindergarten.
Pacific Clinics’ First 5 program works side-by-side with families to build positive relationships between young children (birth to five) and their caregivers and lay the foundation for healthy social and emotional development, which reduces the risk of early childhood problems becoming serious emotional disorders later in life.
Therapeutic Services: Offering emotional support and strengthening family systems by providing child and family-centered therapy to improve communication and enrich relationships.
Home Visitation Services: Providing caregivers with tools to better understand their child’s developmental stages, to foster positive behaviors and to improve family relationships.
Additional Support: Helping to identify family unique needs, linkage to community support/resources such as food banks and housing supports, and hosting parenting and children’s skill-building groups.
The emotional, physical and intellectual environment that children experience before age five has a profound impact on all aspects of their development. By helping very young children and their families start life off with a firm foundation, it is possible to prevent later problems such as abuse, neglect and serious emotional disorders. We welcome the opportunity to partner with our diverse and vibrant communities to support healthy development of children and families.
Pacific Clinics’ FIRST 5 program is part of the Kid Connections Network (KCN) in Santa Clara County.
